Application of C-shaped arm in LARS artificial ligament reconstruction ACL (accurate determination of reconstruction position)

The 2022 Qatar World Cup is underway, and in the first round of Group D group matches on November 24, defending champion France defeated Australia 4-1, but unfortunately, only 11 minutes into the match, France's main defender Lucas Hernandez retired due to injury, and it was confirmed after the match that he had "torn the cruciate ligament in his right knee". He had to regretfully quit the World Cup journey.

The cruciate ligament, also known as the cruciate ligament of the knee, is located in the center of the knee joint and is very strong. It is lined by synovial membrane and can be divided into two ligaments, the anterior and posterior, which work together to limit excessive knee motion.

Soccer and basketball are the most demanding sports for the ACL and the most likely to cause ACL injuries. Many soccer stars around the world have suffered ACL rupture during the game, such as Ronaldo (Brazil), Piero (Italy), James. Owen (England), Deisler (Germany), Diego Costa (Spain), Falcao (Colombia) and so on.

The treatment of ACL rupture was initially a direct repair, but the failure rate was very high. Today, surgical treatment is no longer "repair", but "reconstruction". The most commonly used graft materials are autologous tendons, allograft tendons, and artificial ligaments. In recent years, the recent excellent results of the LARS artificial ligament have been emphasized by the field of arthroscopy and orthopedic sports medicine, and many ball players have used the LARS artificial ligament to reconstruct the ACL.

Application of C-arm in LARS ACL reconstruction

The arthroscope is the key device in ACL reconstruction surgery, hence the term "arthroscopic ACL reconstruction", and the C-arm X-ray machine can also play a role in the LARS ACL reconstruction.

In this procedure, the femoral and tibial stops of the ACL are located, and the tibial and femoral tracts are created respectively. To prevent the extruded screw from prolapsing, care should be taken not to squeeze the screw too deeply during the operation, and it can be squeezed while fluoroscoping under the C-arm machine to ensure that its caudal end is in the lateral bone cortex to effectively avoid squeezing the screw too deeply. Once the caudal end is found to enter the medullary cavity, patients should be informed that they cannot exercise early and start forging 3 weeks after surgery to avoid leading to ligamentous laxity after reconstruction, poor surgical results, and screw retraction leading to nail tract relaxation and fixation failure.

The paper, "The efficacy of arthroscopic combined with LARS artificial ligament under C-arm machine for the treatment of anterior cruciate ligament rupture of knee joint," concluded from the postoperative observation of 46 patients with complete ACL rupture: "Arthroscopic combined with LARS artificial ligament under C-arm machine for ACL reconstruction can effectively restore the stability of knee joint, accurately determine the reconstruction position, and postoperative Autologous ligament growth along the LARS artificial ligament was seen at 12-month MRI follow-up."

PLX7500A, the first flat 3D C-arm in China, is developed and produced by Perlove Medical. The C-arm is equipped with a large-size dynamic flat panel detector of 30cm*30cm, with a pixel size of 154μm, which makes the image delicate and clear, and the gray scale of up to 16 bits also allows the contour of the implant to show a high dynamic range.
